Should you be interested, kindly fill in the application
form (please print and fill) and walk-in to one of our offices
or authorized branches with the copies of the following:
- Passport photocopy
- 12 passport-size photographs (10 for school, 2 for
visa)
- Birth Certificate
- Medical Health Certificate and chest x-ray
- Education certificates / transcript of records
- Criminal clearance report
- Affidavit of support of the person supporting the applicant’s
study
- Financial Statement From Bank regarding capabilities
of the person funding the study of the applicant
- Certificate of good moral character (1 from either
Principal/professor or Dean)
(Items 4-8 need to be authenticated by the Embassy of
the country you are going to)
